Call Sheet (Thibaut Nyemba)

This is the call sheet which we have to fill if you intend to take camera equipment off campus. The person filling it out must include contact details such as phone number and address.  Also emergency contacts and nearest hospital or police station in case if an incident.  Fortunately my group does not have to fill one out as we intend to film on campus.
